My Personhood Summary^© is the intellectual property of the Behavioural Supports Ontario Provincial Coordinating Office (BSO PCO), North Bay Regional Health Centre, Ontario, CANADA.  The tool is intended for use broadly, while preserving its integrity as a standardized tool.  As such, the following permissions are provided and/or required:

Permission is GRANTED from the Behavioural Supports Ontario Provincial Coordinating Office to build My Personhood Summary^© within electronic medical record systems (e.g., Meditech, Cerner, etc.). Please note that the BSO Provincial Coordinating Office has collaborated with PointClickCare on a corporate build of the My Personhood Summary^© Tool as part of the RNAO Clinical Pathway – PointClickCare Nursing Advantage Canada initiative. We are pleased to announce that My Personhood Summary^© is now available in PointClickCare’s for those subscribed to the Nursing Advantage Canada solution.

• BSO’s My Personhood Summary^© is and will remain the intellectual property of the Behavioural Supports Ontario (BSO) Provincial Coordinating Office.
• Alterations to BSO’s My Personhood Summary^© are prohibited. All changes or adaptions to the tool must be approved by the BSO Provincial Coordinating Office. Please contact provincialbso@nbrhc.on.ca to discuss any requested changes to the tool. If the BSO logo is unable to be included due to technical limitations, acknowledgment of BSO as the author of the tool must be stated.
• BSO’s My Personhood Summary^© is a tool made available to users free of charge and as such, individuals/parties must not be charged for the tool or for its individual use.

• Acknowledgement must be given to the BSO Provincial Coordinating Office as owner of the intellectual property of My Personhood Summary^© in any dissemination of research outcomes including, but not limited to, any journal articles, other publications and presentations.
• Research Ethics Board approval has been obtained for your research study (if applicable).
• A copy of published research results must be shared with ProvincialBSO@nbrhc.on.ca.
