The Behavioural Supports Ontario-Dementia Observation System (BSO-DOS^©) is the intellectual property of the Behavioural Supports Ontario Provincial Coordinating Office (BSO PCO), North Bay Regional Health Centre, Ontario, CANADA.  It is intended that the BSO-DOS^© is to be made easily accessible and utilized broadly, while preserving the integrity of this standardized tool.  As such, the following permissions are provided and/or required:

Paper Version (PDF File) - Clinical Use & Educational Purposes

Permission is GRANTED for the paper version of the BSO-DOS^© (available below) to be downloaded, printed, photocopied, distributed and used for clinical use and/or educational purposes.  Alterations to the BSO-DOS^© are prohibited.

Electronic Version Development – Clinical Use

Permission is REQUIRED from the Behavioural Supports Ontario Provincial Coordinating Office to build the BSO-DOS© electronically. This application is intended for electronic vendors or organizations with clinical informatics resources that have interest in building the BSO-DOS© within an electronic documentation system. Research Use
