From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study

Are you an older adult living with dementia, or a caregiver of a person living with dementia, who has experienced a discharge back home from a hospital?

Please consider participating in an interview about your experience. During the interview, you will be asked to answer questions about
  1. the care transition experience going from hospital to home;
  2. any hardships that you experienced since returning home; and
  3. your thoughts around what could be done to make the transition better.
A single interview should take 45-60 minutes, with the potential of a follow-up interview to talk about initial study findings.

The research study, “From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study,” is being conducted by PhD student Marianne Saragosa at the Institute of Health Policy Management and Evaluation at the University of Toronto.

If you are interested in participating in the study, please contact Marianne Saragosa at Marianne.Saragosa@mail.utoronto.ca, or 647-907-1718. For further questions about the study, you can contact the Principal Investigator, Dr. Lianne Jeffs at Lianne.Jeffs@sinaihealth.ca or 416-522-1052.
