​From Hospital to Home
From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study

Are you an older adult living with dementia, or a caregiver of a person living with dementia, who has experienced a discharge back home from a hospital?

Please consider participating in an interview about your experience. During the interview, you will be asked to answer questions about
  1. the care transition experience going from hospital to home;
  2. any hardships that you experienced since returning home; and
  3. your thoughts around what could be done to make the transition better.
A single interview should take 45-60 minutes, with the potential of a follow-up interview to talk about initial study findings.

The research study, “From Hospital to Home for Persons Living with Dementia: A Multi-Perspective Qualitative Study,” is being conducted by PhD student Marianne Saragosa at the Institute of Health Policy Management and Evaluation at the University of Toronto.
