Summary:

The sense of belonging in the elderly is a need that is often overlooked especially with those suffering with dementia. The Love and Belonging education and tools reminds us of the importance to feel that sense of belonging. Building a community in LTC has helped to assist the residents build a sense of community. The outcomes have shown a decrease in physical altercations between residents and a decrease in falls. 
 
By the end of this session, you will
  • Have an understanding of the importance of implementing Love and Belonging education and tools for your staff and residents.
  • Have an understanding of the ADKAR framework to show how it can be effective to implement and sustain change.
  • Review the benefits of implementing an engagement model to build a community in your LTC.

Monica Hebert is a dedicated Registered Practical Nurse with over 32 year's experience providing compassionate, patient-centered care in Long Term Care. Monica worked as an RPN providing direct care for 23 years and then took the position of Assistant Director of Care 10 years ago at Rideaucrest Long Term Care Home.  She is passionate about improving patient outcomes through evidenced- based practices, effective communication and a holistic approach to care. She regularly engages in professional development and strives to stay current with the latest healthcare trends and best practices. 
 
Monica is a GPA certified coach and enjoys training and teaching staff from all departments at Rideaucrest. Monica also has a passion for Wound Care and has been the Wound Care Resource Nurse for the last 20 years. She continues to learn and grow and implement new practices at the home that improves the quality of life for residents.
