After 30 years the PIECES™ Approach is still thriving and helping to transform care with older adults living with complexity.  It continues to be recognized as a best practice in the clinical assessment and care planning.  This webinar will be valuable even for participants with significant familiarity with the PIECES™ Approach. By the end of this session, participants will understand:
  • What makes the PIECES™ Approach unique, as an Assessment framework embedded within a broader Approach, going beyond the PIECES™ Acronym alone.
  • How using the PIECES™ 3-Question Template guides the best possible care for older Persons at risk or living with complex chronic conditions, often involving multiple co-occurring diagnoses and changes in behavioural expression.
Key findings from recent innovative collaborations and research partnerships will also be shared, demonstrating how the PIECES™ Approach can strengthen:
  • Engagement of the Person, Family/Care Partners, and all members of the Team in the shared clinical assessment.
  • Interdisciplinary and intersectoral team collaboration.

Pam Hamilton, BA.H Psychology: Pieces Canada Director
Pam’s career, which spans almost 5 decades including 37 years in senior’s mental health, has been dedicated to improving the lives of older adults living with complexity across health care sectors. She has a depth and breadth of knowledge and experience having worked as a clinician, educator, project coordinator, and developer of innovative evidence informed education initiatives. She is known for her unwavering commitment to supporting a Person and Care Partner centred Team approach through capacity enhancement and the mobilization of knowledge into day-to-day practices. Pam has had the privilege of being involved in the PIECES journey since 1998 and is grateful for the opportunity to have partnered with so many passionate and gifted individuals along the way. As a longstanding member of Pieces Canada and a current Director, she is recognized as a provincial and national leader in the ongoing development, implementation, and evaluation of the PIECES Approach.
 
Joanne Collins, RSW: Pieces Canada Director
Joanne Collins is a Social Worker with more than 30 years of experience in diverse healthcare settings. Throughout her career, she has excelled as a mental health clinician, program manager, and policy analyst. Her work encompasses regional and provincial program development and implementation, as well as leading province-wide strategies for dementia and acquired brain injury. Joanne is highly regarded for her skill in identifying system vulnerabilities, creating innovative interventions, and championing the highest standards of care. Her approach is informed and strengthened by the lived experiences of individuals with cognitive impairment, their families, and healthcare teams working in partnership. As the Managing Director at Pieces Canada, Joanne oversees all aspects of the organization’s operations, including curriculum design and development, and leads all key initiatives that advance the organization's mission. She has integrated the PIECES Approach into her practice, finding her work with Pieces Canada to be a perfect alignment with her skills and values.
