Funded by the Canadian Institutes for Health Research, a three-year research project looked at nine different knowledge translation initiatives carried out by Ontario communities of practice. Several of the initiatives focused on finding ways to improve the care delivered to people living with dementia. In this presentation the research team will share their major findings, including the importance of creating and leveraging relationships, networks, and interactions to bring about change in frontline practice, and the need to design interventions at different levels in the health system. The presentation will include a panel discussion of the results and a Q&A session.
