Lived Experience Network South East Ontario Advisory Zoom Chats

We invite you to join us for the Advisory Lived Experience Zoom chat/conversation. Advisory Lived Experience Zoom Chats/Meetings provide a way for health care system leaders, who plan, assess and provide service to people living with dementia or similar diagnoses in South East Ontario to receive advice and observations from people with lived experience (past and present) with the purpose of improving the system of care. Lived Experience Facilitator, Sharon Osvald will facilitate these Advisory conversations.

Dates & Times:

To accommodate as many people as possible, we have set up two dates to discuss the same topic. One is in the evening, and one is in during the day.
  • Monday, February 3rd Evening Zoom Chat from 6:00 - 7:30 PM       OR
  • Monday, February 10th Daytime Zoom Chat from 1:00 - 2:30 PM

February’s topic:

People living with dementia require a variety of health care services that cross multiple sectors of our health care system. When people cross sectors of the health care system, these are known as transitions in care.  Transitions in care can occur when a person transfers between different care settings (e.g., hospital, primary care, long-term care, home and community care) or between different clinicians. 

Health Quality Ontario suggests people with dementia who transition between settings have a health care team or clinician who is accountable for coordination and communication, ensuring the transmission of complete and accurate information to the family, care partners and receiving health care team prior to the transition. 

Research on transitions tells us that people living with dementia undergo more transitions than other populations and that transitions often lead to adverse events, reduced satisfaction in care, or a potential increase in hospitalization rates.

 From your experience:
  • What does a successful transition look like?
  • What supports are important in a successful transition?  (either that you had or wish you had)
  • What information is important in a successful transition? (for client/care partner; for receiving organization)

The Lived Experience Network South East will hold six virtual advisory conversations a year through Zoom. These Zoom chats/meetings will take place on both a Monday evening and afternoon (with the same topic for both meetings) in the months of February, April, May, September, October and November. Input and suggestions for change will be shared with healthcare professionals providing, planning and evaluating services across the system of care through a quarterly report, in a manner that is confidential and protects your privacy.

Please let us know if you can come, and which date you plan to attend. While it is not mandatory to register, it is very helpful when providing technical assistance, if we have an idea ahead of time of who is coming and so we can provide you with a meeting agenda.

Although, some of our stories are not positive/happy stories, this community is not simply a place to voice our complaints. The Lived Experience Network cannot act as an ombudsman, rather, it is a safe place to share our stories through the lens of hope with the aim of sharing our lived experiences and making things better for others.
  1. Please do not use full names of the people you support. Do not use real names of medical staff, volunteers or the names of hospitals, long term care homes services or organizations. Please keep your comments as general as possible to protect the privacy of yourself and others. Do not share any personal health information about yourself or others. 
  2. Swearing, name calling or comments that are aggressive and assigning blame towards others are not welcome. Participants will be warned and then removed from the Zoom call.
  3. We invite you to take advantage of this online space for the purpose of sharing your insights, stories and suggestions for change, as well as your observations of where you saw good things happening in your care experience. We hope you will feel supported, heard and learn something from each other.
