My Transitional Care Plan^© is the intellectual property of the Behavioural Supports Ontario Provincial Coordinating Office (BSO PCO), North Bay Regional Health Centre, Ontario, Canada & brainXchange^TM, Canada.  The tool is intended for use broadly, while preserving its integrity as a standardized tool.  As such, the following permissions are provided and/or required:

Development of My Transitional Care Plan© in Electronic Medical Record Systems

Permission is GRANTED from the Behavioural Supports Ontario Provincial Coordinating Office & brainXchange to build My Transitional Care Plan^© within electronic medical record systems (e.g., Point Click Care, Meditech, Cerner, etc.). Prior to commencing your build, please review the following Terms of Use and then proceed to the download form.

• My Transitional Care Plan^© is and will remain the intellectual property of the Behavioural Supports Ontario (BSO) Provincial Coordinating Office & brainXchange.
• Alterations to My Transitional Care Plan^© are prohibited. Any changes or adaptions to the tool must be approved by the BSO Provincial Coordinating Office & brainXchange. Please contact provincialbso@nbrhc.on.ca to discuss any requested changes to the tool. If the BSO and brainXchange logos are unable to be included due to technical limitations, acknowledgment of BSO and brainXchange as the authors of the tool must be stated.
• My Transitional Care Plan© is a tool made available to users free of charge and as such, individuals/parties must not be charged for the tool or for its individual use.
