Electronic Version Development – Clinical Use

Permission is REQUIRED from the Behavioural Supports Ontario Provincial Coordinating Office, North Bay Regional Health Centre for the BSO-DOS© to be reproduced, distributed and/or used electronically.  Please complete the following form to request this permission. (*mandatory fields)

Step 2: Please review and accept the ‘Terms of Use’ prior to submitting your request for permission to reproduce, distribute and use the BSO-DOS© electronically.​

Terms of Use Agreement
I, and by extension the organization to which I am affiliated, agree to the following terms of use in the development of an electronic BSO-DOS©:
  • The BSO-DOS© is and will remain the intellectual property of the Behavioural Supports Ontario (BSO) Provincial Coordinating Office, North Bay Regional Health Centre.
  • Any electronic BSO-DOS© will be based on the current version of the tool. The most current version is available for download as a PDF here: www.brainxchange.ca/BSODOS
  • Alterations to the BSO-DOS© are prohibited. Any changes/adaptions to the BSO-DOS© due to electronic application need to be made in consultation with the BSO Provincial Coordinating Office and with their approval.
  • Any electronic BSO-DOS© will be developed according to the Electronic BSO-DOS© Standard provided by the BSO Provincial Coordinating Office. These standards will be provided upon approval of this request.
  • Aligning with the BSO-DOS© Electronic Standard, electronic builds must use point-of-care technology (e.g., direct observations are recorded on a mobile workstation, smartphone, or tablet).
  • Screen shots and/or a prototype demo of any electronic BSO-DOS© will be sent to the BSO Provincial Coordinating Office (ProvincialBSO@nbrhc.on.ca) prior to going live with patients/residents/clients. Confirmation that the developed electronic BSO-DOS© has met the Electronic BSO-DOS© Standard must be granted by the Provincial Coordinating Office prior to the application going live.
  • The BSO-DOS© is a clinical tool made available free of charge to clinicians and organizations as a PDF by the BSO Provincial Coordinating Office, North Bay Regional Health Centre. When built within electronic platforms there is an expectation that the copyrighted tool remains accessible to clinical teams. Any organization/vendor that plans to charge for the use of the approved electronic BSO-DOS© must enter a licencing agreement with the North Bay Regional Health Centre prior to the application going live. Plans related to accessibility of the tool are to be indicated in Step 3 below.
  • The BSO Provincial Coordinating Office, North Bay Regional Health Centre is unable to grant sole development rights to developers.

Step 4: Submit your request.

The BSO Provincial Coordinating Office will consider the request and follow-up via email (within approximately one week) requesting additional information or providing a decision regarding permission to develop the BSO-DOS© electronically.
