BSO is also available to support individuals presenting with, or at risk of, responsive behaviours/personal expression with support to move from one place to another. BSO Teams facilitate transitions in many ways, including:
  • Supporting persons and families to prepare for the move through the development and sharing of relevant information and resources;
  • Organizing/taking part in behavioural care conferences to develop and implement a transitional care plan;
  • Sharing existing behavioural support relevant plans, assessments and other documentation to the team on the receiving end;
  • Supporting the person and family on the day of the move; and
  • Coaching/training the receiving team regarding strategies to best support the person.
